We have nursing staff on duty 24/7 to help our residents with whatever they need, whether that's as simple as assistance getting dressed, or as complex as stroke rehabilitation. We develop a specialized care plan for each resident based on their individual needs. Our facilities have a licensed physician who supervises each resident's care.
When it comes to caring for residents, our nursing staff believes that every person is unique. We get to know our patients' families as well as their personal histories. This helps us give them the personal care they deserve.
Our experienced nursing staff offer the quality medical care you expect. Many have worked in our facilities for years. Also, our small family oriented facility gives our residents a sense of home. We invite family and friends to visit and participate in events, hold birthday parties, and join us on field trips.
We have our own resident therapy dog, Lawton, who seems to know who needs him the most as he goes on his rounds visiting residents in their rooms.
Privacy and Comfort
Our Portola skilled nursing facility was designed to feel like home. Nestled in the pines and planned with quality of life in mind, we've done our best to create a family oriented, warm and friendly atmosphere for our residents.
Both our Portola and Loyalton facilities are small and intimate, unlike the typical large hospital facility. Our rooms are private or double. Residents are encouraged to decorate them with personal items from home. We offer a regularly scheduled barber/beauty shop, a spacious community room, a resident garden, and beautiful surroundings.
We have a Wander Guard system at our Loyalton facility to protect those residents potentially in danger of leaving campus or wandering into unsafe areas. Loyalton also boasts a beautiful, fenced outdoor area with a patio, lawn, and garden.
Quality Medical Care
We offer on-site physical and occupational therapy. Should our residents require additional medical care or treatment, it is immediately available. Our Portola facility is just down the hall from our hospital ER, and we have an ambulance service on-site 24/7 at our Loyalton facility that can transport residents when necessary.
We process most lab work at our Portola campus, and we offer top quality digital imaging services including x-ray, ultrasound, mammography, and CT. Our acute care hospital, literally steps away from our Portola SNF, accommodates most of our residents' needs.

Both our Portola and Loyalton facilities offer skilled comfort care for patients who are facing the end of life. Tahoe Forest Hospice Services highly trained staff strive to ensure our patients' personal dignity is preserved, provide effective pain and symptom management, and support patients and their families physically, emotionally, and spiritually.
